Course description
Immediate implant placement in the anterior maxilla is unforgiving. The facial inclination of the ridge, a thin buccal plate and bundle bone leave little room for error. A straight abutment usually forces a compromise: the implant is placed too facially, the restoration is cemented, or the screw access comes through the labial surface.
This evening lecture presents a protocol that removes that compromise. The Inserta P1 implant is placed on a deliberately palatal axis, in native bone. A Profilo 17° or 30° angulated abutment, seated at surgery and never removed, redirects the screw channel to a cingulum exit. Every case is planned from CBCT and placed through a static surgical guide. This supports immediate placement and immediate provisionalisation while keeping the final restoration screw-retained and retrievable.
Dr Ponnusami also introduces the Esthetic Biological Contour (EBC). This is a way of shaping the transmucosal part of the abutment and crown to give a natural emergence profile. It also keeps the margin and the implant–abutment junction away from the crestal bone. Combined with a one-abutment-one-time approach, it aims to protect the soft-tissue seal and the peri-implant bone.
The session works through the protocol step by step, from case selection and digital planning to the definitive crown. It uses the speaker's own documented cases, including where the technique is not indicated, how to troubleshoot it and how to maintain it. The format is lecture, case presentations and open Q&A. There is no hands-on component.
Who it is for: dentists placing implants who want a predictable anterior single-tooth protocol; restorative dentists working with a surgical colleague; and clinicians considering the Inserta P1 and Profilo system.
Aim & Objectives
Aim: To give dentists a clear, evidence-informed understanding of a digitally guided protocol for immediate single-tooth implant placement in the anterior maxilla. The protocol uses palatal implant positioning with angulated screw-channel abutments, and the course covers its indications, limitations and alternatives.
Objectives. The course will:
- Explain the anatomical constraints of the anterior maxilla, and why straight abutments force a compromise between implant position and screw access.
- Present the rationale for separating the implant axis from the screw-access axis using 17° and 30° angulated abutments.
- Describe the full digital workflow: case selection, CBCT planning, the static surgical guide, atraumatic extraction, guided placement and immediate provisionalisation.
- Introduce the Esthetic Biological Contour (EBC) and the one-abutment-one-time concept for soft-tissue and crestal bone stability.
- Review clinical cases and outcomes, including contraindications, complications, troubleshooting and long-term maintenance.
- Discuss consent, communication with patients and when to refer, so the technique is used within each clinician's competence.
